WE ASK THAT YOU WEAR A FACE-COVER OR MASK FOR IN-PERSON APPOINTMENTS. YOUR TEMPERATURE, SYMPTOM AND COVID-19 EXPOSURE HISTORY WILL BE COLLECTED PRIOR TO CLINIC ENTRY.

CHECK-INS WILL NOW BE DONE REMOTELY VIA A LINK SENT TO YOUR CELL PHONE PRIOR TO YOUR APPOINTMENT. PLEASE UPLOAD YOUR INSURANCE CARD, ANSWER POSED QUESTIONS AND SIGN ALL DOCUMENTS PRIOR TO YOUR APPOINTMENT. THIS IS TO HELP MINIMIZE PHYSICAL INTERACTION WITH FRONT-OFFICE STAFF AND TO PROTECT YOU.
